Should you be away during an assessment deadline, please also complete the CHS Approved Leave Assessment form.
Missed an assessment deadline?
Students are to complete an Illness/Misadventure Application if circumstances beyond their control occur immediately before or during the assessment tasks, affecting their performance for this assessment task.
This may relate to:
- Illness or accident – that is, illness or physical injuries suffered directly by the student which allegedly affected the student performance in the tasks (e.g. influenza, an asthma attack, a cut hand).
- Misadventure – any other event beyond the student’s control, for example the death of a family member or close friend or involvement in a car accident on the way to the school. In such cases students must provide valid documentation.
Please download and complete a form and then submit to the relevant Head teacher with a medical certificate/supporting documentation.

Please download and complete BOTH Illness Misadventure forms and then submit to the relevant Head teacher with a medical certificate/ supporting documentation.
